Vaccination Sign-up Form (Provided at Concordia University, Mequon, WI)

Concordia University is proud to partner with Walgreens Pharmacy to be able to provide vaccines this Fall 2023 on our campus for both our Concordia community and for others in neighboring communities. You do not need to be associated with Concordia to receive vaccines at our campus clinics.

We are currently accepting clinic reservations for the following vaccines:

Some October and November clinics may also include openings for the following vaccines / ages, but their signup is not yet open:

This year you do need to bring your INSURANCE CARD to your clinic appointment (or pay out-of-pocket) in order to receive the vaccine in the clinic. In the past the government provided COVID-19 vaccines free of charge, but this is no longer the case for any vaccine.

We have a team of experienced healthcare providers from medicine, pharmacy, and nursing who look forward to serving you. If you have already signed up and need to change or cancel your time, please click here to send an email. Do not re-submit another duplicate entry to change your time.

The first step in signing up to receive a vaccine at Concordia is to enter below the age of the person who you wish to have vaccinated.
